Question: Minor or major shirk ?
As salam Aleykoum,

Does thinking that making a wish before blowing out the candles on the birthday cake is kufr ?

It is not permissible to celebrate birthdays in Islam as it originates from traditions and practices of other religions.

عن ابن عُمَرَ رضي الله عنهما قال: قال رسولُ الله صلَّى الله عليه وسلم:
«مَن تَشَبَّهَ بِقَوْمٍ فَهُوَ مِنْهُمْ».
(سنن ابي داؤد)
